On 27 February 2024, Viking Therapeutics released results from a Phase 2 obesity trial. Its shares finished the day about 121% higher. On 25 November that year, Cassava Sciences announced that its Alzheimer’s drug had failed a Phase 3 study. Its shares fell roughly 84%. Both companies had experimental medicines, clinical teams and investors hoping for a breakthrough. The results took them in very different directions. ^[1,2]

These are deliberately dramatic examples, not typical returns to expect from a trial announcement. They show why understanding the science is part of understanding the investment. A biotech company can spend years building its case, then publish a few pages of results that force investors to reconsider almost everything they thought it was worth.

If you follow the sector, you see the phase numbers everywhere. A company starts Phase 1. Another reports positive Phase 2 data. A third describes its Phase 3 programme as pivotal. Those labels tell us something about the questions being asked and the evidence available. They do not tell us, on their own, whether a medicine works, whether it will be approved, or whether the shares are attractively priced.

To make sense of the journey, we need to follow the medicine from its first laboratory experiments through human testing, regulatory review and everyday use. Along the way, we also need to understand what companies measure, what they leave uncertain, and what investors may already have paid for. The regulatory examples here focus on the United States and the FDA.

Before Phase 1 there is still a great deal to prove

A treatment usually begins with an idea about biology: perhaps blocking a protein will slow a cancer, replacing a missing enzyme will restore a function, or altering a gene will reduce production of a harmful substance. Researchers test whether a candidate behaves as intended and investigate its potential toxicity. Preclinical work includes laboratory studies and, where appropriate, animal studies. It helps establish whether human testing is justified and informs the starting dose. ^[3]

That is valuable work, but an experimental system represents only part of a human disease. A striking result in cells does not establish that enough medicine can reach the right tissue in a person without causing unacceptable harm. Nor does an animal result settle how a treatment will behave in people taking other medicines or living with several illnesses. For an investor, the unanswered questions at this point are enormous.

In the US, the Investigational New Drug process, usually shortened to IND, is part of getting permission to begin human research. Permission to proceed is permission to investigate, not approval to sell a proven medicine. A trial protocol specifies the participants, treatment, measurements and analysis. Ethical review and informed consent help protect the people who volunteer. They remain people accepting uncertainty, even when a market headline describes them as a catalyst. ^[4,5]

You may also see Phase 0, sometimes called early Phase 1. These optional, very small exploratory studies can examine how a compound behaves in people, sometimes using tiny doses or brief exposure. They help researchers decide what deserves further testing; they are not designed to establish a treatment benefit. Most investors will encounter the familiar Phase 1 to Phase 3 sequence much more often. ^[6]

These are broad teaching ranges, not requirements or deadlines. Rare disease trials can be much smaller; major prevention studies can be much larger. Recruitment, follow-up and the number of studies needed also affect the full development timeline. ^[4,7]

Phase 1 establishes what can sensibly be tested next

Phase 1 usually gives us the first structured human evidence on a new candidate’s safety and dosing. Many programmes recruit healthy volunteers. Cancer trials commonly enrol patients with the disease instead, where the potential risks and possibility of benefit require a different balance. The aim is to learn enough to expose more people responsibly, while watching closely for problems. ^[4]

The terminology can sound more intimidating than the questions. Pharmacokinetics, or PK, concerns how the body handles the drug: how exposure changes over time, for example. Pharmacodynamics, or PD, concerns what the drug does to the body. A useful early study connects the dose to exposure, the intended biological effect and tolerability. You want a dose that can plausibly help, and that people can actually receive. ^[8]

Dose escalation means moving through dose levels under specified safety rules. Expansion cohorts then study selected doses or patient groups in more detail. Finding the highest tolerable dose is not always the same as finding the best dose. The FDA’s Project Optimus specifically encourages oncology development to consider efficacy, safety and tolerability together. More drug may produce more side effects without enough additional benefit to justify them. ^[8]

For investors, early human results can be surprisingly powerful. Intellia Therapeutics and Regeneron reported initial Phase 1 results for NTLA-2001 in June 2021. The treatment used CRISPR technology inside the body to reduce production of transthyretin, a protein involved in ATTR amyloidosis. The initial report covered just six patients. In the three receiving the higher dose, average blood TTR levels fell 87% by day 28. ^[9]

Intellia’s shares rose more than 50% on 28 June 2021. ^[10] My interpretation of that reaction is that investors saw evidence relevant to a wider technology platform, as well as one candidate. A method of editing genes inside the body could potentially underpin several treatments, so an early demonstration could change the value assigned to more than one programme.

Yet an 87% reduction in a protein did not mean an 87% improvement in the disease. Six patients and a short observation period could not settle long-term safety, durability or effects on disability and survival. That is the discipline required with Phase 1: recognise an important scientific advance while being precise about what it has actually established.

The same caution applies to an early cancer response. If two of five patients respond, that is 40%. If one additional patient responds, it becomes 60%. The arithmetic is correct in both cases, but such a small denominator makes the estimate unstable. Before getting excited, ask how many people were treated, how many were included in the analysis, how long they were followed and whether the responses were confirmed.

Phase 2 asks whether the treatment effect is worth pursuing

By Phase 2, the focus increasingly turns to whether the candidate helps patients with the intended condition. Researchers also continue to study safety and refine dosing. The proposed benefit might be fewer migraines, lower body weight, better movement, tumour shrinkage or improvement on a disease-specific scale. The precise question matters more than the phase label. ^[7]

A comparison group helps distinguish the treatment’s effect from changes that would have happened anyway. Randomisation assigns treatment by chance, reducing bias in who receives what. Blinding limits the influence of knowing the treatment assignment. A placebo is an inactive comparator, but placebo use does not necessarily mean withholding standard care: a study can compare a new drug plus standard treatment against placebo plus standard treatment. ^[5]

Consider a hypothetical pain trial. Patients taking the experimental medicine improve by three points. That sounds encouraging until you discover that the placebo group also improves by three points. The question is not simply whether treated patients felt better. It is whether the treatment caused an improvement beyond the comparison group, and whether that improvement was large enough to be useful.

Some studies are open label, meaning treatment assignment is known, or single arm, meaning there is no concurrent comparison group. Such results can still be informative, especially in serious diseases with few options, but comparisons with patients from an entirely different study carry extra uncertainty. Differences in disease severity, previous treatment and follow-up can easily make one candidate look stronger than another. ^[7]

Viking’s VENTURE trial illustrates why Phase 2 can have such a large financial impact. On 27 February 2024, the company reported that its injectable GLP-1/GIP candidate VK2735 produced mean weight loss of up to 14.7% from baseline after 13 weeks. The reported placebo-adjusted reduction reached 13.1 percentage points. Those are different measurements: the latter accounts for the change in the comparison group. The study met its primary and secondary endpoints. ^[11]

The results gave investors a reason to reconsider how competitive the candidate might become, helping explain the roughly 121% share-price rise mentioned earlier. They did not establish that it was superior to another obesity medicine in a direct comparison, or that 13 weeks of results guaranteed the same performance over a year. Longer treatment, larger numbers and the practical effects of tolerability still needed investigation.

You may encounter Phase 2a and Phase 2b. These commonly distinguish earlier exploration from more developed dose selection or efficacy testing, but the naming is not a universal quality scale. Combined Phase 1/2 and Phase 2/3 studies can link stages within one protocol. They do not make the unanswered questions disappear. Read which part of the study produced the result, rather than treating the largest number in its title as a certificate of certainty. ^[12,34]

Phase 3 tests whether the earlier promise holds up

Phase 3 usually provides larger, more definitive evidence for the proposed treatment use. A pivotal trial is one intended to provide key evidence for an approval application; the word describes its role, not a guaranteed result. Study size depends on the disease, the expected effect and what is being measured. An uncommon disease cannot necessarily supply thousands of eligible patients, while a study trying to prevent relatively infrequent events may need very large numbers. ^[7,13]

There are good reasons for a promising Phase 2 result to disappoint later. An early estimate may have been too optimistic. The later trial may recruit a broader population, use a different dose, run longer or measure a more demanding outcome. Background care may improve. A side effect that seemed manageable in a small study may become a bigger problem with longer treatment. These are reasons to scrutinise how the pivotal study differs from the evidence that originally attracted investors.

Cassava’s ReThink-ALZ trial provides a clear example of a late failure. On 25 November 2024, the company reported that simufilam had not met its co-primary endpoints in a randomised Phase 3 trial involving 804 people with mild or moderate Alzheimer’s disease. The study assessed cognition and daily functioning over 52 weeks. The company announced that it would discontinue its other Phase 3 study and its open-label extension. ^[14]

The roughly 84% share-price decline was a reminder that “already in Phase 3” does not mean “nearly certain to work”. Years of development had not removed the need to demonstrate the intended benefit in a controlled trial. For patients and families, the disappointment was far greater than a market loss; for investors, the central commercial assumption had been severely damaged.

There are successes too. Madrigal Pharmaceuticals reported positive Phase 3 MAESTRO-NASH results in December 2022, with resmetirom meeting both primary liver-biopsy endpoints. These assessed resolution of the inflammatory liver disease without worsening fibrosis, and improvement in fibrosis without worsening disease activity. Fibrosis means scarring. The disease was then commonly called NASH and is now called MASH. ^[15]

The FDA granted accelerated approval to resmetirom, branded Rezdiffra, on 14 March 2024 for a specified group of adults with noncirrhotic NASH and moderate to advanced fibrosis. The initial decision relied on biopsy-based evidence, with further work required to verify clinical benefit. That distinction is useful: a Phase 3 programme can support an accelerated approval, and an approval can still leave consequential questions to be answered. ^[16]

Reading the results beneath the headline

An endpoint is the measurement used to judge a trial’s outcome. The primary endpoint is central to the planned test of efficacy. Secondary endpoints answer additional questions. Exploratory analyses can generate useful ideas for future research, but they carry a different evidential weight. A company may find something encouraging even when the trial fails its main test, so begin with what was specified before the results were known. ^[17]

An especially tempting headline reports success in a subgroup after the overall study has disappointed. Perhaps younger patients, one dose or one geographic region looked better. Such findings deserve investigation, particularly when there is a biological explanation. They are also easier to find by chance when researchers make many comparisons. The FDA’s guidance on multiple endpoints addresses this problem: the analysis needs to control the risk of false positive conclusions. ^[17]

A p-value helps assess how compatible the data are with a specified null hypothesis, often that there is no treatment difference. A p-value of 0.03 means that, under that hypothesis and the analysis assumptions, a result at least as extreme would occur with probability 3%. It does not mean there is a 97% chance the medicine works. A threshold such as 0.05 also cannot be interpreted without knowing the trial’s testing plan, including adjustments for multiple comparisons. ^[18]

Statistical significance and clinical usefulness are separate judgements. Imagine a very large trial showing a reduction of one tenth of a point on a symptom scale. It might be statistically convincing yet too small for patients to notice. Conversely, a promising effect in a tiny trial may remain too uncertain to support a firm conclusion. A confidence interval helps show the precision of an estimate: a broad interval should make us less confident about its exact size. ^[32]

Percentages need similar care. In a hypothetical study, an event occurs in 10% of the comparison group and 7% of the treated group over the same follow-up period. That is a 30% relative reduction, but a 3 percentage point absolute reduction. Both are correct. They answer different questions, and the absolute figures help readers understand how many people actually benefited.

Cancer studies introduce several particularly important terms. Objective response rate, or ORR, is the proportion meeting predefined criteria for tumour response. Duration of response asks how long that response lasts. Progression-free survival, or PFS, measures time until progression or death. Overall survival, or OS, measures time until death from any cause. A complete response means no detectable tumour under the assessment criteria; it does not by itself establish a permanent cure. ^[13]

A high response rate is more persuasive when responses last, and a PFS improvement does not automatically establish an OS improvement. A hazard ratio is another comparison frequently reported in survival analyses. An estimated ratio of 0.70 is commonly described as a 30% reduction in the event hazard, subject to the model and its assumptions. It does not mean patients live 30% longer. Always check which event is being measured, the follow-up and the uncertainty around the estimate. ^[13]

Biomarkers need their own distinction. A blood protein, scan measurement or other biological indicator can show that a treatment is affecting its target. A surrogate endpoint is used in place of a direct clinical outcome because it is expected to predict benefit. The strength of that relationship varies: an established surrogate in one setting is not automatically validated for every medicine and disease. This is why moving a laboratory number can be important without settling whether patients feel better, function better or live longer. ^[19]

Finally, read the safety and missing-data tables. Look for treatment discontinuations, dose reductions, serious events and the balance between groups. An adverse event occurring during treatment is not automatically caused by the drug. “Serious” also has a specific meaning, including outcomes such as hospitalisation or death, and is not simply another word for an intense symptom. ^[33] Equally, “generally well tolerated” is an interpretation, not a substitute for the numbers. If the most encouraging efficacy analysis includes only patients who stayed on treatment, ask what happened to those who stopped and how the planned analysis handled them.

Follow-up matters just as much as the headline percentage. A response described as ongoing after two months tells us less about durability than one followed for two years. Interim results are an early analysis, not necessarily the final dataset. “Median not reached” means the data have not yet reached the point needed to estimate that median; it is not a promise that an outcome will never occur. Avoid turning incomplete observation into certainty.

The historical odds are sobering

The BIO, Informa Pharma Intelligence and QLS Advisors report covering 2011–2020 analysed 12,728 clinical and regulatory transitions across 9,704 development programmes. Its historical transition rates were: ^[20]

The estimated overall likelihood of approval from Phase 1 was 7.9%. Applying those rates to an illustrative 1,000 starting programmes gives roughly 520 reaching Phase 2, 150 reaching Phase 3, 87 reaching filing and 79 receiving approval. This is arithmetic using historical rates, not a tracked cohort or a prediction for today’s pipeline.

The filing figure includes resubmissions; it is not a 90.6% chance of approval on the first decision date. The report also found variation: overall approval likelihood from Phase 1 was 5.3% in oncology and 17.0% in rare disease programmes. Programmes using patient preselection biomarkers had a 15.9% likelihood. These are associations across different programme mixes, not proof that adding a biomarker doubles any particular drug’s chances. ^[20]

Use these figures as a starting point for questions. A well-understood mechanism, a reproducible effect and an appropriate patient population may justify a different assessment from a novel mechanism supported by a handful of uncontrolled observations. The generic success rate cannot perform that assessment for you. It also says nothing directly about the price at which a stock becomes attractive.

Approval and Phase 4 bring a different set of questions

After the necessary studies, a sponsor assembles an approval application. NDA stands for New Drug Application; BLA stands for Biologics License Application. The FDA reviews the evidence alongside issues such as manufacturing quality and the proposed prescribing information. A positive trial cannot compensate for every deficiency elsewhere in the application. ^[21]

The PDUFA date investors quote is a target action date, not a guaranteed approval date. A Complete Response Letter, or CRL, means the application cannot be approved in its present form. The practical consequences depend on the deficiencies: correcting a manufacturing issue is very different from conducting another large efficacy trial. Both can cost time and money. The final label also matters because it defines the approved use and may be narrower than investors anticipated. ^[21,22]

Several FDA programmes are often confused. Fast Track facilitates development and review for qualifying serious conditions and unmet needs. Breakthrough Therapy designation involves preliminary clinical evidence suggesting substantial improvement over available therapy and provides additional development support. Neither is a finding that a medicine is already proven or certain to be approved. ^[23,24]

Priority Review concerns the review timetable. The FDA describes a six-month action goal compared with ten months for standard review; applicable review-clock conventions and extensions matter when translating that into a calendar date. Accelerated Approval concerns the evidence supporting an earlier approval, often a surrogate reasonably likely to predict benefit. Required confirmatory work must verify that benefit, and approval can be withdrawn if it does not. These terms should never be treated as interchangeable. ^[25,26]

There is no universal requirement for every medicine to complete three neatly separate trial phases before approval. In some settings, earlier-phase studies can supply pivotal evidence, especially where substantial activity in a serious disease supports an accelerated pathway. Conversely, a Phase 3 label alone does not make a study adequate for approval. The evidence and the proposed use govern the decision. Approval for one patient group also does not automatically establish effectiveness in another. ^[7,13,26]

Research and monitoring continue after a medicine reaches the market. Phase 4 studies can investigate longer-term safety, effectiveness or use. Broader postmarketing surveillance also includes information outside formal trials. More widespread use and longer exposure can reveal problems that the original studies could not detect reliably. Approval establishes a favourable benefit-risk judgement for the authorised use on the available evidence; it does not establish an absence of risk forever. ^[27]

Merck’s Vioxx is a stark historical example. The company voluntarily withdrew it in September 2004 after the APPROVe study identified increased cardiovascular risk during prolonged use. The lesson is not that every approved medicine is hiding a similar problem. It is that evidence continues to accumulate, and a major safety finding can change the judgement about a product already being prescribed. ^[28]

Why the science and the share price do not move together neatly

There is no dependable rule that Phase 1 is worth a 20% rise, Phase 2 a doubling and Phase 3 something larger. Market impact depends on how the result changes expectations, how much of the company depends on the programme and what investors believed before the announcement. The same scientific finding can have very different consequences for a small company with one main asset and a large pharmaceutical group with established sales.

Imagine two companies developing the same hypothetical treatment. One is valued at $200 million and the other at $5 billion. Even if their scientific prospects were identical, the investment proposition would not be. The second valuation requires far more future value to justify what shareholders are paying. A cheap-looking share price is equally unhelpful without the share count: a $2 stock can represent a larger company than a $200 stock.

A simplified valuation example shows how probabilities can affect the arithmetic. Suppose the present value of a programme’s future commercial cash flows, conditional on success, is estimated at $1 billion. Assigning a 20% success probability produces a $200 million probability-weighted figure; raising it to 50% produces $500 million. This is an illustration, not a stock valuation. Development spending, financing, debt, cash, other programmes and the timing of different outcomes still need to be included.

It also explains why a medically positive result can disappoint investors. A study might clear its statistical threshold but show less benefit than expected, a less attractive dosing schedule or more discontinuations. Alternatively, a result could be excellent yet already largely reflected in the valuation. Investors are reassessing both the likelihood of approval and the size of the business that might follow.

Funding is part of that business. A published analysis of 138 pivotal trials supporting 59 FDA-approved medicines in 2015–2016 estimated a median direct trial cost of $19 million, with the middle half between $12.2 million and $33.1 million. This was a historical modelling study of pivotal trials, not a current price list and not the total cost of developing a drug. Discovery, failed programmes, manufacturing and the rest of a company’s operations are separate costs. ^[29]

For a company with $150 million in cash spending $25 million per quarter, simple division gives six quarters of runway. That is only an initial estimate. Spending may accelerate when Phase 3 begins, debts may become due and management will usually want funding in place before cash runs out. Having enough money to announce the next result is different from having enough to finish development and launch.

Success can therefore be followed by a share offering. Viking announced a proposed offering on the same day as its February 2024 results. ^[30] An offering brings money into the company while increasing the share count. It can reduce funding risk and strengthen the programme, even though existing owners hold a smaller percentage. Evaluate what the capital enables and the terms on which it is raised, rather than assuming every raise is either good or bad.

For a simple dilution example, issuing 25 million shares on top of 100 million increases the count by 25%. An investor who owned 1% beforehand would own 0.8% afterwards if they bought no additional shares. Their ownership percentage has fallen by 20%, not 25%. That does not mechanically imply a 20% fall in the share price, because the company has also received cash.

The commercial questions persist after approval. How many patients meet the label? Can the treatment be manufactured and delivered reliably? Will doctors use it, patients remain on it and payers reimburse it? How does it compare with the options available when it launches? A development programme can succeed scientifically while the resulting business falls short of the assumptions embedded in its share price.

What to check before trading a clinical result

Start by writing down what the next announcement can actually establish. First-patient dosing is progress in execution. Completion of enrolment reduces recruitment uncertainty. Neither demonstrates that the drug works. A data release is different, but its usefulness still depends on the study design and how much follow-up is available. Avoid assigning the same weight to every item in a company’s catalyst calendar.

Then read the trial record, the company’s original study description and, when available, the protocol and statistical analysis plan. Compare the reported primary endpoint with the one originally specified. Check the population, comparator, duration and analysis group. A press release is a useful starting point; a full presentation, regulatory review or scientific paper may reveal limitations that the headline cannot fit.

For the investment decision, set out at least three plausible outcomes: convincing success, an ambiguous result requiring more work, and failure. Estimate what each would mean for the remaining pipeline, financing and valuation. Those estimates will be imperfect, but putting them on paper exposes whether the thesis depends on everything going right.

A hypothetical share priced at $10 might be worth $15 after success and $3 after failure. At an assumed 60% probability of success, its simple two-outcome expected value is $10.20: 60% of $15 plus 40% of $3. That is only 2% above the starting price before timing and other complications. A high assumed chance of success has produced a thin expected return because the downside is so severe. Changing the assumptions changes the answer; that is precisely why they should be explicit.

Position size deserves the same attention as the thesis. If a position is 5% of a portfolio and falls 80%, the direct portfolio loss is 4%, assuming everything else stays unchanged. At a 25% position weight, the same fall costs 20% of the portfolio. A stop order cannot guarantee the intended exit price when a stock gaps sharply after news. The amount exposed before the announcement matters. ^[31]

And a good result does not oblige you to buy the first spike. Once the shares have doubled, the entry price and remaining upside have changed. Waiting for the full dataset, a financing announcement, consolidation or a pullback can produce a different decision from buying into the initial excitement. There is no requirement to own every successful medicine’s developer, particularly at a price that leaves little room for disappointment.

When the next biotech headline arrives, try to describe it without the company’s adjectives. State what was tested, in whom, against what, for how long and with what result. Then say what remains unknown and what the current valuation assumes. If you can do that clearly, you are in a much better position to decide whether the announcement has changed the investment case, or simply made an already expensive possibility sound more exciting.

FDA decisions are often presented as simple, binary events.

Approved means the stock rises. Rejected means it falls.

In practice, biotech rarely works that neatly. A company can receive approval and lose 20% of its value within hours. Another can win an approval that most investors already expected and still jump 25%. A rejection caused by a manufacturing problem may be recoverable, while a rejection questioning whether the drug works can destroy most of the investment case.

The FDA decision itself is only part of the equation. The market is also reacting to what it expected, the wording of the label, the reason for any rejection, the commercial opportunity and how much of the company’s value depended on that single drug.

A study covering thousands of pharmaceutical announcements found that negative events generally produce stronger market reactions than positive ones. It also found something familiar to experienced biotech investors: stocks sometimes fall after apparently positive news.

The following ten decisions from 2024 to 2026 show why.

The ten decisions at a glance

1. Replimune: sometimes the final FDA decision is not the main event

Replimune’s melanoma treatment, now called Tudriqev, had already been rejected twice before returning to the FDA in 2026.

The central problem was not whether the treatment produced any responses. It was whether a single-arm study could establish how much benefit came from RP1 itself when it was used alongside Bristol Myers Squibb’s Opdivo. FDA reviewers questioned the lack of a control group and produced a more conservative assessment of the response data.

Those concerns sent Replimune shares down by more than 30% when the FDA’s briefing documents appeared.

Two days later, an advisory committee voted 10 to 3 in favour of the evidence. The stock more than doubled. By the time the FDA formally approved the treatment, much of the regulatory value had already been added to the share price.

This is an important pattern. The PDUFA date may be circled on everyone’s calendar, but the decisive moment can arrive earlier through briefing documents, an advisory committee vote or updated FDA correspondence.

Anyone waiting for the final “approved” headline may find that the market has already moved.

2. Moderna: approval does not solve a timing problem

Moderna’s mFlusiva became the first mRNA-based seasonal flu vaccine approved in the United States. For adults aged 50 to 64, it received traditional approval. For those aged 65 and over, the FDA used accelerated approval based partly on immune-response data.

It was an important validation of Moderna’s technology outside COVID-19.

The stock initially rose by around 4% in premarket trading, but later slipped by roughly 3%. The reason was not that the approval lacked scientific significance. It was that investors had already started looking beyond the headline.

An advisory committee had voted unanimously in favour of the vaccine, making approval increasingly likely. Moderna had also missed important purchasing and contracting cycles, limiting the product’s near-term commercial contribution. Analysts were not expecting meaningful revenue until later in 2027.

The company received the approval investors wanted, but not an immediate solution to its revenue problem.

For larger biotech companies, the value of an FDA approval depends on how quickly the product can affect revenue, cash flow and the broader investment case. Scientific progress and financial progress do not always arrive together.

3. Rocket Pharmaceuticals: an approved drug can still be difficult to sell

Rocket Pharmaceuticals received FDA approval for Kresladi, the first approved gene therapy for severe leukocyte adhesion deficiency type I.

The disease is devastating. Without effective treatment, many affected children die at a very young age. Kresladi therefore addressed a genuine unmet medical need and also earned Rocket a valuable priority review voucher.

Rocket shares initially rose by more than 10%. By later in the session, they were down approximately 15% to 20%.

Investors quickly shifted their attention from approval to delivery. Severe LAD-I is extremely rare. Patients must be identified, referred to specialist centres, prepared for gene therapy and supported through a complicated treatment process. Rocket planned a phased launch through a small number of centres, with commercial availability not beginning immediately.

Approval established that Kresladi could reach the market. It did not establish how quickly Rocket could find patients, secure reimbursement and convert the approval into meaningful revenue.

A small patient population can support a valuable rare-disease drug, particularly when pricing is high. It can also produce a slow and uneven launch. Approval removes regulatory risk, but it does not remove commercial friction.

4. Omeros: low expectations can make approval explosive

Omeros had spent years trying to secure approval for narsoplimab, eventually marketed as Yartemlea, for transplant-associated thrombotic microangiopathy.

The FDA initially rejected the application in 2021. The company continued discussions with the agency, provided additional analyses and remained committed to the programme, but investor confidence deteriorated. By late 2025, the drug had become a “show me” situation. The market wanted a decision, not another regulatory update.

When approval finally arrived, Omeros shares rose nearly 70%, having briefly more than doubled intraday.

The treatment became the first FDA-approved therapy for the condition, covering adults and children aged two and older. The approval also transformed Omeros from a company pursuing a disputed regulatory case into one with a commercial product.

This was not simply the market valuing a new drug. It was the market removing years of accumulated doubt.

Biotech stocks can become so discounted after repeated setbacks that eventual success produces a much larger move than the size of the immediate commercial opportunity might suggest. The share price is not just reflecting the drug. It is reflecting how many investors still believe the company can deliver anything at all.

A deep dive into Afrezza, Furoscix, Tyvaso DPI, debt, valuation, upcoming catalysts and the trading opportunity around 26 July.

MannKind is often described as the company behind Afrezza, the inhaled insulin that has spent years trying to break into a market dominated by injectable products.

That description is now badly out of date.

MannKind has become a strange but interesting hybrid: part specialty pharmaceutical company, part drug-manufacturing partner, part royalty business and part clinical-stage biotech. It owns commercial products, receives recurring income from United Therapeutics, carries substantial debt and is developing several new inhaled treatments through its Technosphere platform.

That complexity is probably one reason the market struggles to value it.

At a share price of approximately $4.03, MannKind is valued at around $1.24 billion. According to the TipRanks screen dated 17 July, analysts have five Buy ratings, one Hold and no Sell ratings, with an average target of $8.15. But the immediate reason traders are watching the stock is much closer: an FDA decision on the new Furoscix ReadyFlow autoinjector, with a target date of 26 July 2026.

MannKind at a glance

What does MannKind actually do?

MannKind now has three principal commercial engines.

The first is Afrezza, an ultra-rapid inhaled mealtime insulin. The second is Furoscix, an at-home treatment for fluid overload associated with heart failure and chronic kidney disease. The third is its partnership with United Therapeutics, under which MannKind manufactures Tyvaso DPI and receives manufacturing income and royalties.

It also owns the smaller V-Go insulin-delivery business and is developing inhaled versions of drugs for pulmonary fibrosis, pulmonary hypertension and fluid overload.

A useful way to think about MannKind is:

Afrezza and Furoscix provide the owned-product opportunity. Tyvaso DPI provides the recurring financial foundation. The pipeline provides the speculative upside.

The quality and risk of these revenue streams are very different.

Where the revenue comes from

MannKind reported $90.2 million of revenue in Q1 2026, an increase of 15% from the previous year.

Afrezza sales increased only 3%, while Tyvaso DPI royalties increased 9%. Collaboration and manufacturing revenue declined 20%, partly because this revenue can move around depending on United Therapeutics’ ordering and production requirements. Furoscix contributed $15.5 million following MannKind’s acquisition of scPharmaceuticals in October 2025.

Approximately 62% of MannKind’s Q1 revenue came from United Therapeutics when Tyvaso royalties, manufacturing and licence revenue are combined.

That concentration is both MannKind’s greatest strength and one of its largest risks.

United Therapeutics gives MannKind a growing commercial product without MannKind having to fund the entire sales operation. But it also means a large proportion of revenue depends on one partner, one franchise and decisions MannKind does not directly control.

The Tyvaso DPI engine

Tyvaso DPI is an inhaled dry-powder formulation of treprostinil used for pulmonary arterial hypertension and pulmonary hypertension associated with interstitial lung disease.

United Therapeutics handles development and commercialisation, while MannKind manufactures the product and receives royalties.

United Therapeutics reported $330.3 million of Q1 2026 Tyvaso DPI sales, up 9% year over year. MannKind records a contractual royalty equal to 10% of Tyvaso DPI sales, although it previously sold the economic right to one percentage point of that royalty. It therefore retains the economic benefit of approximately 9%.

This is arguably MannKind’s best asset.

Royalty revenue generally requires much less incremental selling expenditure than revenue from a drug MannKind must market itself. As Tyvaso DPI sales grow, a significant proportion of the additional royalty income can potentially flow toward operating profit and cash flow.

However, investors should understand the accounting.

MannKind still reports the full 10% royalty as revenue and records payments connected with the sold royalty interest separately. The associated royalty liability stood at approximately $150.6 million at the end of Q1. This is not a conventional loan requiring immediate repayment, but I would still treat it as a debt-like obligation when making a conservative enterprise-value calculation.

The longer-term IPF opportunity

United Therapeutics has reported positive Phase III results from the TETON programme evaluating nebulised Tyvaso in idiopathic pulmonary fibrosis. In TETON-1, the reported difference in forced vital capacity was 130.1 mL in favour of treatment, with a highly statistically significant result.

United Therapeutics intends to submit an additional application for the nebulised product by the end of summer 2026.

This could eventually benefit MannKind, but there is an important detail: an expanded indication for nebulised Tyvaso would not automatically give Tyvaso DPI the same indication. United Therapeutics has indicated that bridging studies and potentially additional pivotal work may be required for the dry-powder version. It is therefore valuable longer-term optionality, rather than an immediate expansion of MannKind’s royalty base.

Afrezza: an attractive idea that still needs commercial proof

Afrezza is inhaled at the beginning of a meal and acts more rapidly than conventional injected mealtime insulin.

The product’s obvious appeal is convenience. Some patients dislike injections, while Afrezza’s rapid action and relatively short duration may help certain patients manage post-meal glucose without insulin remaining active for as long.

On 29 May 2026, the FDA expanded Afrezza’s approval to include children aged six and older with diabetes. MannKind describes Afrezza as the first and only inhaled insulin approved for paediatric use in the United States. The company estimates that more than 350,000 US children and adolescents are living with diabetes.

That sounds like a major commercial opportunity, but approval does not guarantee rapid adoption.

Afrezza has been available for adults for years, yet Q1 sales were still only $15.3 million. For the paediatric launch to become meaningful, MannKind must persuade doctors, patients, parents, insurers and diabetes educators that the benefits justify changing established treatment routines.

There are also important limitations:

• Patients with Type 1 diabetes must still use long-acting basal insulin.

• Afrezza is contraindicated in patients with chronic lung diseases such as asthma or COPD because of the risk of acute bronchospasm.

• Lung-function testing is required before starting treatment, again after six months and annually thereafter.

• It is not recommended for smokers or people who have recently stopped smoking.

• Diabetic ketoacidosis occurred more frequently in Afrezza-treated Type 1 diabetes patients in clinical studies than in comparator-treated patients.

None of these limitations makes Afrezza a bad product. They simply help explain why a needle-free insulin has not automatically captured a huge share of the diabetes market.

For investors, the paediatric approval should be judged through actual commercial evidence:

1. New prescriptions.

2. Repeat prescriptions and patient retention.

3. The number of doctors prescribing the product.

4. Insurance coverage and prior-authorisation requirements.

5. Discounts and rebates required to generate sales.

6. Whether revenue growth eventually exceeds the additional sales expenditure.

The next few quarterly reports will be more important than the approval headline itself.

Furoscix: the product behind the July catalyst

Furoscix is a subcutaneous formulation of furosemide, a powerful diuretic used to remove excess fluid.

Patients with worsening heart failure or chronic kidney disease can become overloaded with fluid. Oral diuretics may sometimes become insufficient or inconsistently absorbed, while intravenous furosemide usually requires treatment in a medical facility.

Furoscix attempts to fill the space between those two options by delivering an IV-equivalent dose at home.

The existing Furoscix on-body infusor administers treatment over approximately five hours. MannKind’s new ReadyFlow device is a handheld autoinjector designed to administer the dose in less than ten seconds.

In a bioavailability study, ReadyFlow produced estimated bioavailability of 107.3% relative to intravenous furosemide, with the confidence interval remaining inside the conventional bioequivalence range.

If approved, ReadyFlow could be far easier for patients and clinicians to use than a five-hour wearable device. MannKind has also said that patent protection could potentially extend through 2040.

Furoscix generated $15.5 million in Q1 revenue, while doses dispensed increased 64% year over year. That provides evidence that the franchise already has commercial traction before ReadyFlow enters the market.

ReadyFlow is not entering an empty market. The competitive picture should not be ignored.

The FDA has also approved Lasix ONYU, another subcutaneous furosemide product for adults with chronic heart failure. Like the existing Furoscix device, Lasix ONYU delivers the drug through an on-body system over approximately five hours.

ReadyFlow’s main potential advantage is therefore not that it is the only way to administer furosemide outside a hospital. Its advantage is the possibility of delivering an equivalent dose in seconds rather than hours.

That is a compelling distinction, but commercial success will still depend on:

• The final FDA label.

• Reimbursement and patient out-of-pocket cost.

• Whether cardiologists and nephrologists change their prescribing behaviour.

• Whether the device is genuinely easy for older or unwell patients to use.

• Manufacturing capacity and product availability.

• Evidence that at-home treatment reduces emergency visits or hospital admissions.

• Competition from oral, intravenous and other subcutaneous formulations.

One hidden cost of a ReadyFlow approval

MannKind acquired scPharmaceuticals in October 2025, paying $5.35 per share in cash and issuing contingent value rights to former scPharmaceuticals shareholders.

Under those arrangements, ReadyFlow approval by 30 September 2026 can trigger a payment of $0.75 per CVR. A later approval would produce a smaller payment, while additional CVR payments are linked to Furoscix and ReadyFlow sales milestones.

The CVR liability had an estimated fair value of approximately $29 million at the end of March.

This does not undermine the approval thesis, but it is something traders may miss.

A ReadyFlow approval would create commercial value while also creating a cash obligation. The press release may therefore generate excitement, while the subsequent financial statements reveal an associated payment.

The often-quoted $110 million to $120 million Furoscix sales range also needs to be described correctly. It appears in the acquisition’s CVR milestone terms. It should not automatically be presented as formal management revenue guidance.

The pipeline beyond commercial products

MNKD-201

MNKD-201 is MannKind’s inhaled formulation of nintedanib, an established oral treatment for idiopathic pulmonary fibrosis.

Oral nintedanib can be difficult for some patients to tolerate because of systemic adverse effects. MannKind’s research hypothesis is that direct delivery to the lungs may produce useful pulmonary exposure while reducing exposure elsewhere in the body.

That remains a hypothesis—not a proven clinical advantage.

MannKind completed the first cohort of its Phase 1b INFLO-1 study without reported serious adverse events or discontinuations. Topline data are expected during Q3 2026. The company has also started moving the programme toward the Phase II INFLO-2 study.

The Q3 readout should be interpreted primarily as a safety and pharmacokinetic event. Investors should not expect an early study to establish that the product reduces disease progression.

The useful questions will be:

• Did inhaled dosing produce adequate drug exposure in the lungs?

• Was systemic exposure lower than with oral treatment?

• Were gastrointestinal adverse effects reduced?

• Did patients tolerate repeated dosing?

• Is the selected dose practical for a later-stage study?

MNKD-1501

MNKD-1501 is an inhaled dry-powder version of ralinepag being developed with United Therapeutics for pulmonary hypertension.

MannKind received an upfront payment and can receive additional development milestones, followed by a 10% royalty on a resulting product. The programme adds another potential partner-funded product to the Technosphere platform.

MNKD-701

MNKD-701 is a preclinical inhaled formulation of bumetanide being explored for fluid overload associated with heart failure and chronic kidney disease.

The logic is similar to Furoscix: deliver a potent diuretic without requiring an intravenous visit. But it remains early and should not receive much value in a conservative investment model yet.

One useful research lesson is that pipeline webpages can become stale. MannKind’s previous clofazimine programme was discontinued in Q4 2025, which is why the latest 10-K, 10-Q and earnings release should take priority over an older investor presentation or pipeline graphic.

The fundamentals: growth is real, but so is the spending

MannKind generated $349.0 million of revenue in 2025, up 22%.

The largest components were:

• Tyvaso DPI royalties: $128.1 million, up 25%.

• Collaboration and service revenue: $106.7 million, up 6%.

• Afrezza: $74.6 million, up 16%.

• Furoscix: $23.2 million following the October acquisition.

• V-Go: $16.4 million, down 10%.

Q1 2026 revenue then grew another 15% to $90.2 million.

However, MannKind is spending heavily to launch and support its expanded portfolio. Q1 research and development expense increased 56% to $17.2 million, while selling, general and administrative expense increased 116% to $54.1 million.

The higher SG&A spending reflects the Furoscix organisation, the scPharmaceuticals acquisition and preparations for the paediatric Afrezza launch.

This is the central fundamental question:

Can revenue grow quickly enough for MannKind’s royalty income and commercial products to absorb the expanded cost base?

A company can report impressive revenue growth and still destroy value if every additional dollar of sales requires even more spending.

For the operating model to work, SG&A growth must eventually slow while Furoscix, Afrezza and royalty revenue continue rising.

Net loss is not the same as cash burn

MannKind reported a Q1 GAAP net loss of $16.6 million, or $0.05 per share. Its non-GAAP adjusted loss was approximately $6.9 million, or $0.02 per share.

However, the company used only $5.4 million of cash in operating activities during the quarter.

Why the difference?

Net income contains non-cash items such as depreciation, amortisation, stock compensation and changes in the value of financial liabilities. Cash flow measures the money that actually entered or left the business during the period.

The reverse can also happen: reported earnings may look acceptable while cash disappears because receivables rise, inventory is built or liabilities are paid.

That is why biotech and pharmaceutical investors should always examine at least three figures together:

1. Net profit or loss.

2. Operating cash flow.

3. Total movement in cash after investing and financing activity.

MannKind’s cash balance fell by much more than its $5.4 million operating burn because it also used approximately $35.5 million to settle the remainder of its convertible notes. That was a financing transaction rather than ordinary operating expenditure.

The real cash runway

MannKind ended Q1 with approximately $133.9 million in cash, cash equivalents and investments.

Using the Q1 operating cash burn of $5.365 million:

$133.9M ÷ $21.46M annualised burn = approximately 6.2 years.

Including Q1 capital expenditure of $1.88 million:

$133.9M ÷ $28.98M annualised cash use = approximately 4.6 years.

That produces the headline runway range of approximately 4.6 to 6.2 years.

But there is another level of analysis.

MannKind’s debt agreement requires it to maintain at least $40 million of liquidity at quarter-end. That means the entire $133.9 million should not be treated as freely spendable.

Subtracting the required $40 million gives approximately $93.9 million of headroom:

• Operating-burn basis: approximately 4 years.

• Operating burn plus capital expenditure: approximately 3 years.

This covenant-adjusted range is more conservative and, in my view, more useful.

Even that is only a snapshot. It does not include the eventual repayment or refinancing of the $325 million term loan, potential CVR payments, acquisition-related costs or future changes in commercial and clinical spending. Conversely, it also does not assume that revenue and cash generation improve.

MannKind itself states only that its available resources are expected to be sufficient for at least the next 12 months. Companies generally avoid giving the kind of multi-year runway calculation investors often make themselves.

The debt matters

MannKind has $325 million of principal outstanding under its Blackstone term loan.

The loan matures in August 2030 and is largely repayable at maturity rather than gradually amortising. Its effective interest rate was approximately 9.09% at the end of Q1. It is secured against substantially all the company’s assets, including intellectual property.

At a 9.09% effective rate, $325 million represents a rough annual interest burden of around $29.5 million from the term loan alone.

MannKind paid $9.7 million of cash interest across its obligations during Q1. That means the company must generate considerable cash merely to service its capital structure before reducing principal or returning anything to shareholders.

The company may have access to additional financing, including a delayed-draw commitment and potentially larger uncommitted facilities. But access to more debt is not the same as having more equity value. Borrowing can extend liquidity while increasing future interest and refinancing risk.

This is why I would not describe MannKind as financially weak, but I would also not describe the balance sheet as clean.

Near-term dilution does not appear to be the obvious base-case threat. The more important financial risks are debt service, the 2030 maturity, contingent acquisition payments and whether the commercial business reaches sustainable positive cash flow.

Valuation

At approximately $4.03 per share, MannKind’s market capitalisation is around $1.24 billion.

Using $325 million of term-loan debt and $133.9 million of cash and investments produces a conventional enterprise value of approximately:

$1.24B + $325M − $133.9M = $1.43B

Against FY2025 revenue of $349 million:

• Market capitalisation to sales: approximately 3.6 times.

• Conventional enterprise value to sales: approximately 4.1 times.

Treating the $150.6 million royalty liability as debt-like produces an adjusted enterprise value of approximately $1.58 billion, or roughly 4.5 times FY2025 revenue.

Those multiples are not obviously cheap for a loss-making company, but a simple revenue multiple does not capture the business properly.

A more sensible valuation would separate:

1. The high-margin Tyvaso DPI royalty stream.

2. Tyvaso manufacturing and collaboration income.

3. The Afrezza franchise.

4. Furoscix and the potential ReadyFlow launch.

5. The clinical pipeline.

6. Cash.

7. Debt, royalty obligations and contingent payments.

The Tyvaso royalty stream deserves a higher multiple than a labour-intensive commercial product because it requires less selling expenditure. Afrezza and Furoscix should be valued on expected future cash flow, not simply headline sales. The pipeline should be heavily probability-adjusted because most early clinical programmes do not reach approval.

What analysts think

According to the TipRanks screen supplied for this research:

• Five analysts rate MannKind a Buy.

• One rates it a Hold.

• None rate it a Sell.

• The average target is $8.15.

• The high target is $11.

• The low target is $4.75.

At a $4.03 share price, the average target implies approximately 102% upside.

Recent published targets include an $11 target from Wells Fargo and a $9 target from Truist, while RBC’s $4.75 target and Hold rating demonstrate that not every analyst sees a major rerating ahead.

Analyst targets can be useful for understanding expectations, but they should not be mistaken for probabilities.

A target usually reflects assumptions about future sales, margins, regulatory outcomes and valuation multiples. When any of those assumptions change, the target changes. An analyst’s $11 scenario also does not protect a trader from a sharp decline following a regulatory disappointment.

The catalyst calendar

24–27 July 2026: ReadyFlow FDA watch window

The official FDA target date is Sunday, 26 July.

Because the date falls on a weekend, traders should be alert from Friday 24 July through Monday 27 July. The FDA can also act ahead of a PDUFA date or, occasionally, communicate later. The date is a target, not a guaranteed announcement appointment.

This is the immediate binary catalyst.

Q3 2026: MNKD-201 Phase 1b results

MannKind expects topline INFLO-1 data for inhaled nintedanib during Q3.

The most important information will be tolerability, pulmonary exposure, systemic exposure and the dose selected for Phase II—not proof of clinical efficacy.

Second half of 2026: paediatric Afrezza launch evidence

Investors should watch prescription growth, payer coverage, the number of new prescribers and whether Afrezza revenue accelerates from the modest 3% growth reported in Q1.

End of summer 2026: United Therapeutics IPF submission

United Therapeutics intends to seek an expanded indication for nebulised Tyvaso following the positive TETON programme. This is an indirect and longer-term MannKind catalyst because the DPI formulation would still require additional development work for the same indication.

Next quarterly report

The next earnings report will provide an important update on Furoscix sales, commercial spending, cash burn and early paediatric Afrezza indicators. I have not found a confirmed reporting date in the primary sources reviewed, so it is better not to present an estimated calendar date as official.

The ReadyFlow trading opportunity

ReadyFlow creates a classic event-driven biotech setup: a defined regulatory date, a product with existing clinical and commercial foundations, and a share price that could respond sharply to the wording of an FDA decision.

But “FDA approval” is not a single uniform outcome.

Bullish outcome

The strongest possible announcement would contain:

• Full approval by or before the target date.

• A commercially useful label covering the expected heart-failure and kidney-disease populations.

• No unexpected safety restrictions.

• Immediate or near-immediate product availability.

• Clear reimbursement and launch plans.

• Confidence that supply can meet demand.

• Constructive guidance on switching patients from the existing infusor.

That combination could cause investors to raise Furoscix revenue estimates and assign more value to the scPharmaceuticals acquisition.

Mixed outcome

The FDA could approve ReadyFlow but leave investors underwhelmed because:

• The label is narrower than expected.

• Launch timing is delayed.

• Manufacturing or device supply is limited.

• Pricing or reimbursement remains unclear.

• Management avoids giving commercial guidance.

• Approval was already heavily anticipated by short-term traders.

That could produce a muted reaction or a “sell the news” move despite the positive headline.

Bearish outcome

A Complete Response Letter, material delay or unexpected request for additional data would probably trigger a negative repricing.

The severity would depend on whether the issue is:

• Easily correctable manufacturing documentation.

• A device or human-factors issue.

• A request for additional clinical data.

• A broader safety or efficacy concern.

The press release headline will not tell the whole story. Traders should read the explanation of what the FDA requested and how long management expects remediation to take.

Three ways traders can approach it

1. Trade the run-up but exit before the decision

This approach seeks to capture pre-catalyst interest without accepting the full regulatory outcome risk.

The disadvantage is that the stock may not run up, or the FDA could announce its decision earlier than expected.

2. Hold a deliberately small position through the decision

This provides full exposure to a positive gap but also full exposure to a negative one.

Position sizing matters far more than confidence. A sensible binary-event position is one where a severe adverse gap would be painful but not damaging to the overall portfolio.

3. Wait for the result

A trader can wait for the FDA outcome, read the label and launch details, then trade the post-event breakout or subsequent pullback.

This sacrifices some potential upside but removes much of the regulatory uncertainty. It may be the cleaner approach for anyone whose real thesis is commercial adoption rather than the regulatory event itself.

A warning on spread betting

A leveraged spread bet makes binary catalyst risk significantly more dangerous.

FDA news can arrive outside market hours and cause the stock to reopen far above or below the previous price. A normal stop-loss may be executed at the next available price rather than the level selected, meaning the actual loss can be much larger than expected.

For an FDA event, leverage should not be mistaken for conviction.

What to read when the decision arrives

Do not trade solely from the words “FDA approves.”

Read the complete announcement and look for:

1. The exact indication.

2. Eligible patient populations.

3. Dosing and administration requirements.

4. Safety warnings and monitoring.

5. Whether the product is available immediately.

6. Manufacturing and inventory readiness.

7. Reimbursement and payer plans.

8. Pricing relative to the current Furoscix device.

9. Management’s updated commercial expectations.

10. The resulting CVR payment and effect on cash.

Those details will determine whether approval materially changes future revenue forecasts.

How to research a biotech properly

MannKind is a useful case study because reading only the investor presentation produces an incomplete picture.

Start with the 10-Q and 10-K

The filings show debt terms, customer concentration, contingent obligations, cash flow, acquisition accounting and commitments that may receive little attention in marketing materials.

Separate revenue by quality

A dollar of royalty revenue is not economically identical to a dollar of product revenue.

Ask who pays the selling costs, who controls the product, whether the revenue is recurring and how much direct expenditure is required to generate the next dollar.

Reconcile profit with cash

Look at net income, operating cash flow, capital expenditure and financing activity separately. MannKind’s Q1 cash decline would look alarming without recognising that $35.5 million went toward settling convertible notes.

Calculate more than one runway

A proper runway analysis should include:

• Cash divided by operating burn.

• Cash divided by operating burn plus capital expenditure.

• Available cash above debt covenants.

• Debt maturities and likely refinancing needs.

• Contingent payments.

• Whether revenue is growing quickly enough to reduce future burn.

Read the FDA label

A press release highlights the opportunity. The FDA label reveals who can actually use the product, contraindications, required monitoring and warnings that may limit adoption.

Research the partner

Because United Therapeutics generates approximately 62% of MannKind’s revenue, its filings and earnings calls are essential MannKind research.

A MannKind investor who never reads United Therapeutics’ results is missing a large part of the thesis.

Check ClinicalTrials.gov

Look at trial design, enrolment, endpoints, comparison groups, estimated completion dates and whether the study status has changed. Company timelines are useful, but the trial registry provides a second source.

Read acquisition agreements

The ReadyFlow CVR shows why acquisition terms matter. A successful catalyst can create an asset while simultaneously triggering a cash payment.

Use analyst targets last

Build the thesis first. Then compare it with analysts to identify where your assumptions differ.

The bull case

The strongest MannKind argument is that the company has finally built enough scale and diversification to move beyond the old Afrezza-only story.

Tyvaso DPI provides growing royalty and manufacturing income. Furoscix already has commercial momentum, with ReadyFlow potentially making the franchise much easier to use. Afrezza now has access to a paediatric population. MNKD-201 and the broader inhaled pipeline offer additional upside.

Cash burn was modest in Q1 relative to the available cash balance, and immediate equity dilution does not appear necessary under the current trajectory.

If revenue keeps growing while SG&A stabilises, MannKind could move from a loss-making hybrid into a genuinely cash-generative specialty pharmaceutical company.

The bear case

The balance sheet is heavily leveraged, with $325 million of term debt, expensive interest and a 2030 maturity that will eventually require repayment or refinancing.

United Therapeutics accounts for approximately 62% of revenue, leaving MannKind exposed to one partner and one product franchise.

Afrezza’s commercial history shows that an attractive medical concept does not always produce rapid adoption. Furoscix and ReadyFlow must compete with established oral and IV furosemide as well as another FDA-approved subcutaneous device.

SG&A has more than doubled, meaning strong product growth is required simply to absorb the enlarged commercial organisation.

ReadyFlow approval may already be partly priced in, while a regulatory setback could produce an immediate negative gap.

My conclusion

MannKind is much more interesting than the old “inhaled insulin company” description suggests.

The Tyvaso DPI partnership has created a valuable recurring revenue stream. Furoscix gives the company a credible second owned franchise. ReadyFlow could make that franchise considerably more useful, while paediatric Afrezza and MNKD-201 add further optionality.

But this is not a simple undervalued biotech with a huge cash pile.

MannKind carries substantial debt, unusual royalty obligations, contingent acquisition payments and a rapidly expanded cost base. Its reported runway looks comfortable at approximately 4.6 to 6.2 years, but the covenant-adjusted cushion is closer to 3.2 to 4.4 years, and the $325 million loan maturity in 2030 cannot be ignored.

For traders, the 26 July ReadyFlow decision is the immediate opportunity.

For longer-term investors, approval is only the beginning. The real proof will come from prescription growth, reimbursement, commercial margins and whether MannKind can convert its enlarged product portfolio into sustainable positive cash flow.

At approximately $4.03, the stock offers meaningful upside if the bull case develops—but it also carries enough regulatory, commercial and balance-sheet risk that position sizing matters far more than any analyst price target.

Viking Therapeutics $VKTX has spent years being discussed as a takeover candidate. That may eventually happen, but it is not a sensible investment thesis on its own.

At roughly $39 a share, Viking is valued at around $4.5 billion. After subtracting its $603 million cash and investment balance, the market is assigning approximately $3.9 billion to its pipeline and corporate infrastructure. This is no longer an overlooked biotech trading around cash. Investors are already paying a meaningful amount for clinical success.

The reason is VK2735.

Viking has developed a dual GLP-1 and GIP receptor agonist, targeting the same two receptors as Lilly’s tirzepatide. What makes Viking particularly interesting is that it is developing VK2735 as both a weekly injection and a daily tablet. It may therefore be able to offer patients different formulations without having to build two unrelated drugs.

That is the attraction. The difficulty is proving that the results seen in relatively short Phase II trials will hold up across thousands of patients receiving the drug for more than a year.

The injection is the real investment case

In Viking’s 13-week Phase II VENTURE study, weekly injectable VK2735 produced average weight loss of as much as 14.7% from baseline and up to 13.1% after adjusting for placebo. Weight loss had not reached an obvious plateau when the study ended.

At the highest dose, as many as 88% of participants lost at least 10% of their body weight, compared with 4% receiving placebo. Those are exceptionally strong results for a study lasting only three months.

The tolerability also looked relatively manageable. Around 13% of patients receiving VK2735 discontinued treatment, compared with 14% in the placebo group. Nausea was reported by 43% of treated patients and vomiting by 18%, with one serious adverse event involving dehydration considered related to treatment.

It is important not to compare percentages mechanically across different obesity trials. Patient populations, starting weights, dose escalation and study length can all distort comparisons. Nevertheless, the Phase II injection data were strong enough to justify moving directly into a large Phase III programme.

VANQUISH-1 has enrolled approximately 4,500 adults with obesity or who are overweight with at least one related condition. Patients are receiving one of three maintenance doses—7.5 mg, 12.5 mg or 17.5 mg—or placebo for 78 weeks, followed by an extension period.

VANQUISH-2 has enrolled approximately 1,000 patients with obesity or overweight and type 2 diabetes. It uses the same three doses and also runs for 78 weeks. Enrollment in both trials is now complete, with the principal Phase III readouts expected in 2027.

This is where the real company-making risk sits. A 13-week Phase II trial can show that a drug is active. A 78-week Phase III programme reveals whether people can remain on it, whether efficacy continues to build and whether uncommon safety issues begin to appear.

Should VANQUISH reproduce the Phase II profile, Viking would possess one of the most valuable unpartnered metabolic assets in biotechnology. Should discontinuations rise materially or weight loss flatten earlier than expected, the present valuation would become much harder to defend.

The oral tablet works, but it is not yet clean

The oral version of VK2735 has produced impressive weight-loss figures of its own.

In the 280-patient VENTURE-Oral Phase II study, the highest 120 mg dose produced average weight loss of 12.2% after 13 weeks, equivalent to approximately 26.6 pounds. The placebo-adjusted reduction was 10.9%. As many as 97% of patients achieved at least 5% weight loss, while up to 80% lost at least 10%. Once again, the weight-loss curve had not clearly plateaued.

That is the positive part of the oral story.

The problem was tolerability. Across the active treatment groups, 20% of participants discontinued because of adverse events, compared with 13% on placebo. Total discontinuations reached 28% on VK2735 versus 18% on placebo.

At the highest 120 mg dose, 38% discontinued treatment early. Nausea affected 60% of patients in that group and vomiting 35%, although the company said gastrointestinal events were generally concentrated around the beginning of treatment and declined over time.

This does not make the oral drug a failure. It means the dosing schedule requires work.

Viking can use a lower starting dose, slower escalation and possibly a lower maintenance dose in Phase III. An exploratory part of the Phase II study also suggested that patients who were stepped down from 90 mg to 30 mg maintained most of their weight loss, finishing 9.2% below baseline. That may prove useful when designing a longer and more tolerable regimen.

Following its end-of-Phase-II meeting with the FDA, Viking expects to begin an oral Phase III programme in the fourth quarter of 2026. The design will matter almost as much as the eventual result. A carefully titrated tablet producing slightly less weight loss with materially fewer discontinuations could be commercially more useful than chasing the highest possible dose.

For now, I would treat the injection as the core asset and the oral formulation as meaningful upside. An investment case that requires the 120 mg oral dose to succeed exactly as tested is unnecessarily fragile.

The most interesting 2026 catalyst may not be Phase III

Viking now treats maintenance as a distinct part of its obesity franchise rather than simply an extension of the pivotal programme. On the company’s official pipeline, the maintenance study sits alongside the injectable and oral obesity programmes as its own development programme.

The Phase I study is enrolling approximately 180 adults. Participants first receive 19 weeks of weekly injectable VK2735 before being randomised to different maintenance strategies, including weekly, every-other-week and monthly injections, as well as weekly and daily oral dosing. Results are expected during the third quarter of 2026.

This isn’t designed to prove efficacy in the same way as VANQUISH. Instead, it explores how patients might realistically stay on treatment after achieving meaningful weight loss. If Viking can demonstrate that patients maintain most of their weight while reducing injection frequency or switching to oral therapy, it would create a much broader commercial franchise rather than simply another weekly obesity drug.

Obesity treatment is increasingly likely to become a long-term market rather than a short course of therapy. A considerable number of patients regain weight after stopping treatment. Cost, injection fatigue, gastrointestinal side effects and the inconvenience of weekly dosing may therefore become as important as the maximum amount of weight lost.

Suppose a patient could use the weekly injection for the initial weight-loss phase and then maintain most of that loss with a monthly injection or weekly tablet. Viking would have something more differentiated than another GLP-1 competing solely on headline efficacy.

The study is small and exploratory, so it cannot prove a commercial maintenance strategy on its own. But a convincing result would give Viking a more coherent franchise: intensive treatment, oral treatment and lower-frequency maintenance, all built around the same molecule.

Viking is also building a second obesity programme

In June, Viking began a first-in-human study of VK3019, an internally developed dual amylin and calcitonin receptor agonist.

The initial Phase I trial is testing single subcutaneous doses in adults with a BMI of at least 30, primarily examining safety, tolerability and pharmacokinetics. Viking has reported encouraging weight reduction in preclinical animal studies, but VK3019 remains far too early to assign substantial value to it.

The strategic logic is still clear. Amylin-based drugs may eventually be used alone, in combination with incretin therapies or as maintenance treatments. Novo Nordisk, Lilly, Roche and others are all investigating mechanisms beyond conventional GLP-1 drugs.

VK3019 gives Viking another way to participate if the obesity market begins moving towards combinations. It also makes the company less dependent on a single scientific mechanism, although it will be several years before investors know whether the molecule is genuinely differentiated.

VK2809 should have been a valuable second asset

Outside obesity, Viking has generated strong data with VK2809, an oral thyroid hormone receptor beta agonist being developed for metabolic dysfunction-associated steatohepatitis, or MASH.

In the Phase IIb VOYAGE study, up to 75% of patients achieved MASH resolution without worsening of fibrosis, compared with 29% receiving placebo. As many as 57% achieved at least a one-stage improvement in fibrosis without worsening of MASH, compared with 34% on placebo. The treatment was also reported to have an adverse-event profile broadly similar to placebo.

Those results would ordinarily make VK2809 an important part of Viking’s valuation.

There is now a serious complication.

In April 2026, Ligand Pharmaceuticals purported to terminate the licence covering Viking’s thyroid hormone receptor beta (TRβ) programme, including VK2809 and VK0214. Viking disputes the validity of the termination and says it intends to defend its contractual rights.

Until that dispute is resolved, I would assign limited value to VK2809. Losing the licence would remove what appeared to be Viking’s most advanced non-obesity asset. Even a negotiated settlement could introduce new payments, royalties or development obligations.

The dispute does not appear to threaten VK2735. Viking reports owning the intellectual property surrounding its GLP-1/GIP programme, including US and international patents and pending applications. The Ligand issue is therefore an overhang on the secondary pipeline rather than an existential threat to the central obesity programme.

Viking also retains two smaller programmes that receive far less investor attention. VK0214 is an oral thyroid hormone receptor beta (TRβ) agonist for X-linked adrenoleukodystrophy (X-ALD), where a Phase Ib study demonstrated proof of concept through reductions in very-long-chain fatty acids. However, like VK2809, it forms part of the ongoing licensing dispute with Ligand Pharmaceuticals.

VK5211 is a selective androgen receptor modulator (SARM) designed to preserve lean muscle mass. It remains part of Viking’s pipeline but appears to be a lower strategic priority than the obesity franchise. Unlike VK2809 and VK0214, VK5211 is not affected by the Ligand dispute.

Neither programme currently drives the investment case.

The balance sheet is strong, but not as strong as it first appears

At approximately $39 a share, Viking Therapeutics is valued at around $4.5 billion. The company held $603 million in cash, cash equivalents and short-term investments at 31 March 2026, leaving a rough cash-adjusted valuation of approximately $3.9 billion for the pipeline and business.

Viking spent $150.2 million on research and development during the first quarter and used $114 million of operating cash. That quarterly outflow should not be treated as a fixed burn rate, because the timing of clinical invoices, manufacturing commitments and prepayments can produce considerable variation. It nevertheless demonstrates how expensive the VANQUISH programme has become.

Management believes its existing resources will fund operations through at least 30 June 2027. Viking also states plainly that it will require additional capital to complete its ongoing and planned trials. The company had $63.7 million of capacity remaining under its existing at-the-market programme at the end of March, but a larger financing, partnership or other strategic transaction may eventually be required.

Dilution is therefore not a remote risk. It is part of the likely financing path.

That does not necessarily make the shares unattractive. Raising several hundred million dollars would be manageable if Phase III data substantially increased the value of VK2735. The danger is being forced to raise during a weak biotech market or after an inconclusive clinical update.

Viking is preparing as though it may commercialise the drug itself

Viking has signed broad manufacturing agreements with CordenPharma covering both injectable and oral VK2735.

The planned capacity includes multiple metric tonnes of active pharmaceutical ingredient each year, up to 100 million autoinjectors, another 100 million vial or syringe products and more than one billion oral tablets annually. The agreements can also be expanded.

This is a significant commitment for a company with no commercial products. It suggests Viking is not simply conducting enough development work to attract a buyer. Management is attempting to preserve the option of launching VK2735 itself or entering a partnership from a stronger negotiating position.

The agreements also create financial and execution risk. Viking is making prepayments against future manufacturing orders, while remaining dependent on third parties to produce commercial-scale quantities that meet regulatory standards.

Manufacturing obesity medicines is not a minor detail. Novo Nordisk and Lilly have spent billions expanding capacity, and shortages have repeatedly constrained the market. A successful drug that cannot be produced reliably is not a successful commercial product.

Competition will be brutal

Viking is entering a market already dominated by two of the largest pharmaceutical companies in the world.

Lilly and Novo Nordisk have approved drugs, established prescriber relationships, global reimbursement teams and enormous manufacturing programmes. Behind them is an expanding group that includes Amgen, Roche, Pfizer and several smaller biotechnology companies developing oral drugs, longer-acting injections, amylin combinations and muscle-preserving treatments.

The eventual obesity market may exceed $100 billion in annual sales, which means Viking would not need to beat every competitor to create substantial value. Even a small share could support several billion dollars of revenue. But a large market does not automatically produce attractive economics for every participant. Competition could force discounts, higher rebate payments and expensive head-to-head studies.

VK2735 therefore needs to establish a reason to exist beyond being another effective weight-loss drug.

That reason could be unusually rapid initial weight loss. It could be an effective oral formulation. It could be monthly maintenance. It could be a favourable tolerability profile at lower doses. At present, Viking has several possible points of differentiation, but none has yet been proven in a registrational trial.

What is the market assuming?

At the current price, Viking’s valuation is neither obviously excessive nor obviously cheap.

The market is assigning substantial value to VK2735, but nowhere near the value of a fully approved, commercially validated obesity franchise. That leaves room for the shares to rise considerably if Phase III succeeds, while also leaving plenty to lose if the programme disappoints.

Analysts remain overwhelmingly optimistic. The current average 12-month target compiled by TipRanks is approximately $92.91, with targets ranging from $70 to $125. That implies around 139% upside. H.C. Wainwright has maintained a $102 target, while Truist has used $83.

These targets are useful as a measure of sentiment, not as evidence of intrinsic value. Most assume successful development of VK2735 and some probability of a partnership or acquisition. They offer little protection if the pivotal programme fails.

In a bullish outcome, injectable VK2735 reproduces its Phase II efficacy, discontinuations remain manageable, the maintenance study demonstrates flexible dosing and the oral programme advances with an improved titration schedule. Viking could then become a serious independent obesity company or one of the most obvious large-scale acquisition targets in biotechnology.

In a more moderate outcome, the injection succeeds but proves broadly similar to other late-stage drugs, the oral formulation remains constrained by gastrointestinal side effects and Viking raises additional capital to fund commercial preparation. That could still support a valuable company, although perhaps not the spectacular outcome implied by the highest analyst targets.

In the bearish outcome, Phase III weight loss regresses towards the middle of the class, discontinuations increase over 78 weeks and Viking needs to raise money before confidence in the programme has recovered. The Ligand dispute could simultaneously remove the MASH programme from the valuation.

The conclusion

Viking is one of the more credible independent obesity developers, but the stock should not be treated as a cheap buyout ticket.

The injectable version of VK2735 is the reason to own it. Its Phase II performance was strong enough to justify the excitement, and enrollment of two large Phase III trials has been completed impressively quickly.

The oral version is promising but needs a better tolerability profile. VK3019 is interesting but extremely early. VK2809 has good clinical data but is trapped inside a licensing dispute. The company has enough cash to continue operating, although probably not enough to reach the complete Phase III outcome and build a commercial organisation without further financing.

The Q3 maintenance data could be the most important near-term signal. Monthly injections or weekly oral maintenance would give Viking something more strategically interesting than another once-weekly incretin.

At approximately $4.5 billion, investors are paying for a genuine probability of success, but not yet for category leadership.

Viking does not need to replace Lilly or Novo Nordisk. It needs to prove that VK2735 can occupy a durable place beside them.

That remains possible.

It is simply much harder than producing 13 weeks of excellent weight-loss data.
